WASHINGTON, July 14, 2014 /PRNewswire/ -- Healthcare providers need to efficiently acquire accurate and detailed health information from patients at the time-of-care. However, health data that are available from a patient's memory and assorted records are often inaccurate, incomplete and fragmented. A website,, addresses this problem by enabling patients to create and continuously update accurate and consistently structured Health History reports. Providers could increase efficiency by quickly reviewing, assimilating and selecting relevant data from these reports shared as print-outs or via online interaction.

The Web site offers free encrypted portals for patients to anonymously enter data using a computer, tablet or smart phone. Patients' researched past health history plus new data entered in real-time could be dated and described in a dialog box before automatic placement in the database. Reports could be maintained as files in the database, but an editable copy should be printed or downloaded to a secure environment before adding personal ID. Also, data that patients considered too sensitive for processing via the Internet could be added to blank spaces that had been saved in appropriate positions while online.

Providers could establish their own free portals that would enable them to use temporary identification, furnished by a patient, to access the patient's database files and create Work Sheets. The main purposes of this set of tools are the creation and maintenance of a History of Present Illness report in which the patient's Chief Complaint could be integrated chronologically with related health topics in the patient's database files. Providers might also choose to create and maintain a Work Sheet for the Physical Examination, Evaluation, and/or Management sections of a patient's traditional legal Medical Record.

Anyone with basic word-processing ability and access to the Internet should be able to create, update, save and retrieve a Health History report without cost, divulging identity, or need to process potentially harmful information online. Health reports, edited by a provider to meet standard criteria, could serve as the updating documents for paper medical records. Staff could cost-effectively transfer the provider-selected data from reports into electronic medical records (EMR). Providers can increase their productivity when time saved, by acquiring and documenting patients' data efficiently, is used to interact with more patients.
