PHOENIX (3TV/CBS 5) -

A child has died following a serious crash involving three adults and another child in north Phoenix Monday night.

That 3-year-old boy has been identified as Noah Perry.

The crash happened near 18th Street and Greenway Parkway.

Phoenix police say that an SUV with a family of four inside was driving eastbound when it collided with another car with one person inside driving westbound.

The force of the collision sent the vehicles off the roadway and into a block wall around a residence, police said.

All passengers except for the driver of the SUV were ejected and suffered life-threatening injuries.

A 3-year-old from the vehicle was transported to the hospital where he later died. Police say the other child, a 6-year-old girl, is currently listed in critical condition.

"It's heartbreaking," said neighbor Martina Anderson. "My daughter is only a month older than the little boy. We are going to miss his presence. He's a sweet little boy."

Police say that the 33-year-old woman's condition is listed as critical but stable.

Meanwhile, a 36-year-old man and 20-year-old man both suffered non-life-threatening injuries.

According to Phoenix police, no child seats were recovered at the scene.

Police also say the children were not wearing seatbelts.

"When these ejections take place, it suggests that the occupants were not properly restrained," said Phoenix police Sgt. Vince Lewis. "We are talking a 3-year-old and a 6-year-old. There should be some child restraint systems deployed in this situation and they weren't."

Impairment is being investigated as a possible factor in the crash.

Greenway Parkway was closed for several hours from 20th to 16th streets.
