Sales of the plate benefit a trauma center at a Memphis hospital named after the rock 'n' roll singer.
But if at least 250 more fans don't order those tags, the design could be scrapped.
The Regional Medical Center needs a required 1,000 purchasers to justify the mass production of the design, which shows the King in a white suit strumming a white guitar.
Hospital officials say they need those last 250 fans to buy the plate before the June 30 deadline.
The hospital is hoping a series of radio ads in Nashville, Knoxville and Chattanooga will convince those last few fans.
The hospital estimated it would receive roughly $20 in revenue for each plate sold.
